Submitting is free of charge. No one may be treated unfavourably for having made a submission.
Only the authorised people responsible for the review can access the information in your submission. Anonymous submissions are not asked for a name or contact details.
Anonymous submissions go through the same steps; we simply cannot reply because we have no contact details. If you would like a reply, leave an email address or phone number in the form.
Your submission is recorded under a reference number, which is shown on screen once you submit.
Within 2 business days, your submission is reviewed and assigned to the responsible person.
Within 10 business days, a resolution or the current status is determined.
If you left contact details, we let you know the outcome through the channel you chose. If you are not satisfied with the response, you can submit again quoting your reference number.
In an emergency that threatens life or safety, call 112 first.
